Such large amounts of body fat are the norm for hippos, so there is nothing hipocritical about a hippo advising someone else to watch their weight, unless it's another hippo that has a lower or higher then normal amount of body fat , in which case - lol.

If he suffered from diabetes (you know because of his weight) and he didn't eat enough or took too much insulin he would suffer from hippo-glyceamic shock and possibly go into a coma and die

hippothetically a hippodermic syringe with insulin should help, and no diabetic hippos dont respond well to counselling or hipponosis. :P

If he became a doctor, he'd have to take a hippo-cratic oath. Then he could treat all those hippo-chondriacs and diabetics having hippo-glycemic episodes.

And then refuse to take his own advise and die.
